COMMUNITY HEALTH NURSE

COMMUNITY HEALTH PROGRAM PERMANENT FULL – TIME POSITION DEPARTMENT OF HEALTH

PAY LEVEL: RN01

SUMMARY OF RESPONSIBILITIES;

Working within the context of a multidisciplinary public health team, the Community Health Nurse (CHN) promotes the health of individuals, families and community members. The CHN is responsible for; client education, counselling, care coordination, develops, implements and evaluates public health programs and services. The CHN follows Best Practice Guidelines and Public Health Standards to; promote health, build individual and community capacity, facilitate access and equity, demonstrate professional responsibility and adheres to Nursing Code of Ethics. The CHN will be asked to specialize in specific areas.

QUALIFICATIONS;

• Must have proof of good standing with the College of Nurses of Ontario and/or Quebec with a BScN OR a Diploma nurse with more than 5 years’ experience in the field of nursing

• Must obtain both nursing licenses within six (6) months of hiring

• Must obtain tobacco cessation certification and Health Canada Competencies completed within six months of hiring

• Must have valid CPR/First Aid certification

• Must have extensive and proven experience in program development (5 years)

• Must be an effective public speaker and have experience leading group presentations

• Must be able to lift 20 lbs. and work flexible hours and some evenings or weekends

• Must be eligible to drive company vehicles according to MCA FLEET management policy (with at least five years driving experience and a clean driver’s abstract)
