PERMANENT FULL-TIME POSITION COMMUNITY HEALTH PROGRAM

 


Annual Salary: RN 01 $61,544.40 – $80,799.53

SUMMARY OF RESPONSIBILITIES:

Working within the context of a multi-disciplinary public health team, the Community Health Nurse (CHN) promotes the health of individuals, families, community members. The CHN is responsible for client education, counseling, care coordination; develops, implements and evaluates public health programs and services. The CHN follows Best Practice Guidelines and Public Health Standards to promote health, building individual and community capacity, facilitate access and equality; demonstrates professional responsibility and adheres to Nursing Code of Ethics.

QUALIFICATIONS:

Must have a Baccalaureate Degree (BScN) in Nursing from a Canadian University, a certificate or Diploma in Nursing from a Recognized Canadian College with certification in Public Health / Community Health. Must have proof of good standing with the College of Nurses of Ontario and/ or Quebec or be eligible to apply. Must be willing to successfully complete any other training that is requested from Supervisor.

Must have a good understanding of Community and Public Health issues; must have current certification in First Aid and CPR. Must have good computer skills. Ability to speak Mohawk is an asset.
