MEDICAL SOCIAL LIAISON WORKER
PART TIME POSITION HOME CARE HOME SUPPORT PROGRAM DEPARTMENT OF HEALTH SALARY LEVEL: PT MHT ST 2 $75,000.26
SUMMARY OF RESPONSIBILITIES:
Under the direction of the Home Care Home Support Program Manager, the incumbent’s priority is to provide clients, families or vulnerable sector with the social and psychosocial support needed to cope with acute, chronic or terminal illnesses. This includes direct services with clients that may involve providing resources, counselling and therapy sessions, advocating for their needs, crisis intervention, research and development and assistance in community development and/or policy development. The incumbent will lead the Multidisciplinary team members to include Home Care Home Support team, primary care providers and specialized programs and services.
QUALIFICATIONS:
• Masters in Social Work, Psychology or related field with a clinical concentration from a university of recognized standing; Bachelor’s Degree may be considered.
• Must be registered or eligible and a member in good standing with the College of Social Work of Ontario and Ordre Professionnel des Travailleurs Sociaux du Quebec (College of Social Work in Quebec) or eligible for a permit.
• Minimum of three (3) years’ experience in a clinical setting.
• Experience in a supervisory role an asset.
• Current First Aid/CPR.
• Knowledge and experience with First Nations culture considered an asset.
