CASE MANAGER

PERMANENT FULL-TIME POSITION AKWESASNE CHILD & FAMILY SERVICES

 


Salary Level:  HS 12 - Salary Under Review

 SUMMARY OF RESPONSIBILITIES: 

Under the supervision of the Case Management Unit Coordinator, the incumbent is responsible for conducting client intake, strength and needs assessment, service planning, referrals and monitoring of the service provision until closure of the file.

QUALIFICATIONS:

Bachelors Degree in Social Work preferred, Bachelors in related field accepted. Minimum educational qualifications; Two years College Diploma in the Social Service or equivalent; (Law & Security, Child and Youth Worker, Social Work, Behavioral Science and Criminal Justice, Psychology) and minimum 2 year work experience in social work field. 

The incumbent must be familiar with the legislation, regulations and guidelines as determined by agency requirements in working with children and youth.  Must possess effective and excellent communication skills; and must adhere to strict principles of the Oath of Confidentiality.  

Must be willing to work flexible hours i.e. evenings and weekends; and be able to work the on – call schedule, work effectively in decision making situations. Ability to speak the Mohawk Language considered an asset. Must have a class G drivers license and be willing to use own personal vehicle during the course of work.
